Students new to District 39 need to have an Illinois medical and dental form on file. These forms must be completed and returned to the Health Office by October 15 or your child WILL be excluded from school.
The school nurse will be performing hearing and vision screenings during the month of October (10/1, 10/8, 10/15, and 10/22). Students who wear or have glasses are reminded to bring them to the screening. Parents will only be notified if a problem is detected.
